OverviewThe Rehab Admission & Marketing Representative is responsible for providing case management services to all patients referred to WakeMed Rehab as well as marketing activities to monitor and develop relationships with all referral sources. This includes conducting a thorough clinical evaluation, assessing available resources and facilitating the development of a discharge/transfer plan from the referral location to the appropriate program within WakeMed Rehab system, or recommendations for alternative care options. This position is responsible for managing the planning, development and maintenance of an efficient admission system, establishing a community referral system and maintaining positive public relations with all internal and external contacts.
